CBD: The Next Weapon In The War Against Opioid Addiction?

CBD, or cannabidiol, is everywhere, with word on the street saying that it can cure everything from a bad mood to cancer. However, most of these claims are not based on scientific evidence. Animal studies suggest that CBD might be beneficial for some health indications, such as pain, inflammation, arthritis and anxiety.

However, until recently, the only medical indication that CBD has been proven to treat in humans is seizures associated with pediatric epilepsy. Now, however, a recent study suggested that CBD curbed cravings in people with opioid dependence. This is one of the first double-blind controlled trials, the gold standard for drug research, to show benefit of using CBD outside epilepsy treatment. Thus, researchers can say with greater confidence that CBD may be helpful in fighting the war against opioid addiction.

While this study is very exciting, as scientists who study drugs and addiction, we want to stress that this study was very narrow and used specific, standardized amounts of CBD. Thus, the results do not suggest that buying a bottle or jar of over-the-counter CBD is going to help with opioid cravings – or any other medical conditions.

Addiction is a brain disease

In order to understand why CBD might be useful to treat opioid addiction, it is helpful to take a closer look at how addiction alters normal behavior. Addiction is broadly defined by the American Psychiatric Association as “a complex condition, a brain disease that is manifested by compulsive substance use despite harmful consequence.” Addiction is classified as a disease because addiction hijacks and alters the way how the brain processes information.

Specifically, areas of the brain critical in controlling the perception of daily and pleasurable activities are susceptible to the influence of addictive drugs. Due to the rewiring of the brain under addiction, the individual often perceives the world in context to their drug of choice. The brain learns to associate drug paraphernalia or the physical location of drug partaking in the context of receiving a drug. These cues become integral reminders and reinforcers of drug use.

These events occur with most known drugs of abuse, such as cocaine, alcohol, nicotine, methamphetamines as well as opioids.

Addiction is often thought of in terms of the pursuit of the “high” associated with the use of a drug. However, most addicts continue to use, or relapse when trying to quit using their respective addictive drug. This difficulty, despite the desire and often pressure by friends, family and co-workers to quit, is often due to the negative effects of drug withdrawal.

Depending on the drug, the symptoms of drug withdrawal can vary and range from mild to severe intensity. In the case of opioid withdrawal, symptoms often include anxiety, nausea, vomiting, diarrhea, abdominal cramps and rapid heartbeat. An individual going through opioid withdrawal experiencing extreme conditions of anxiety is likely to take opioids to alleviate that anxiety. This sort of behavior can be repetitive, leading to a what is called a feed-forward loop of dependence on an abused drug.

A person is often referred to as “dependent” on a drug when the drug must be present for the individual to function normally. Importantly, anxiety and depression are correlated with opioid dependence.

For dependent individuals, ongoing use of a drug is not perceived as a conscious choice, but rather an evil necessity. Medication-assisted treatment with drugs like methadone or buprenorphine, allows for an individual to undergo recovery from an opioid use disorder. The use of medication assisted treatment significantly decreases the likelihood of an individual to relapse and fatally overdose due to withdrawal or dependence symptoms.

CBD and Epidiolex

CBD was tested in several clinical trials and was shown to work and to be safe in treating a rare form of epilepsy. A pharmaceutical grade CBD, Epidiolex, gained FDA approval in June 2018 for this specific usage.

CBD is currently only prescribed as the drug Epidiolex. That is because, up until now, CBD has only been shown to be safe and effective in the treatment of intractable pediatric epilepsy.

Importantly, CBD binds to different receptors than those that lead to opioid addiction.

CBD and opioid addiction

In experiments reported in 2009, rats were trained to press a lever to receive heroin. CBD did not decrease the amount of heroin that the rats self-administered, or the drug seeking behavior displayed by the rat while taking heroin. However, when rats were taken off heroin and given CBD, there was a decrease in drug-seeking behavior when the animals were exposed to a heroin-associated cue.

Initial studies of CBD in humans verified that CBD, when co-administered with fentanyl, is safe and well tolerated in healthy, non-opioid dependent individuals. A 2015 report of a small double-blind study conducted in opioid-dependent individuals found that a single administration of CBD, in comparison to a placebo, decreased cue-induced craving of opioids and feelings of anxiety. A double-blind, placebo-controlled study means that doctors and patients in the study do not know who is getting a real drug and who is getting a placebo. That is to guard against what is known as the placebo effect.

A double-blind placebo-controlled study published on May 21, 2019 adds to these findings by demonstrating that the Food and Drug Administration-approved Epidiolex can reduce cue-induced craving in individuals that had been former heroin users. Furthermore, in these individuals, Epidiolex reduced reports of anxiety, and blood levels of cortisol, a hormone known to increase under conditions of stress and anxiety.

Although further studies are needed, these studies strongly suggest that Epidiolex or CBD may hold promise as a critical weapon in fighting the opioid epidemic.

Practical considerations

Before rushing out to purchase over-the-counter CBD to treat any medical condition, there are several practical considerations that should be considered.

Only Epidiolex is FDA-approved for a medical condition – pediatric seizures. All other forms of CBD aren’t regulated. There have been numerous consumer reports that show that the actual amount of CBD in over-the-counter products is significantly less than what is reported on the label. Also, some of these over-the-counter products contain enough THC to show up on drug tests.

Although Epidiolex was found to be safe in clinical trials, it can interact with other drugs prescribed for migraines and bipolar disorder. This could mean that taking CBD with certain drugs could diminish or enhance the effects of prescriptions, leading to problems controlling particular medical conditions that were once well-managed, or increase side effects of the other medications. For this reason, it is incredibly important to talk to your doctor or pharmacist about potential drug interactions before using CBD.The Conversation

Martin, who is a longtime friend of Markle’s, as well as the Dior Beauty brand ambassador, helped her achieve her wedding day look. But he said she’s always pretty much had the same beauty routine.

“She likes a stronger eye, her brow is a lot more defined now. But it’s still her. If anything, she’s going to experiment with different tones, and now that she’s more tan she’ll use warmer colors. But she doesn’t stray too far from her technique, she’s very formulaic with her routine.”

And that includes wearing nude lip colors in lieu of bold ones, like red. Said Martin:

“The one time she did a red lip, she just didn’t feel comfortable in it. She likes to talk and she’s not a fussy person, so she doesn’t want to have to worry about anything.”

Martin said Markle will continue to do her own makeup even while she’s abroad on royal assignments.

“They have so many engagements, you have to stick with what you know and feel comfortable and confident and just own it,” he says, adding that Markle’s wedding makeup started a “less is more” beauty trend; people loved that you could still see her freckles and that they weren’t covered up with heavy foundation.

“It’s about owning your own beauty,” he said. “We’ve been inundated on social media with contouring and highlighting and it’s created this narrative that you need so much makeup to be beautiful, and I think that day when they saw her, it was just her looking beautiful with not a lot on. They embraced her rather than her face first, and that was the intention.”

TSA Updates Rules To Allow Hemp-Derived CBD Aboard Flights

Over Memorial Day weekend, the Transportation Security Administration updated its policy on marijuana, specifying that some hemp-derived CBD products may be carried aboard flights if the situation fits certain circumstances.

“Products/medications that contain hemp-derived CBD or are approved by the FDA are legal as long as it is produced within the regulations defined by the law under the Agriculture Improvement Act 2018,” explains the new section on the site. The 2018 Farm Bill was significant because it legalized hemp products and derivatives.

Previously, the TSA made no distinction between different cannabis products, claiming that none of them could be carried aboard flights, neither in carry on items or in checked in bags.

CNN reports that although some cannabis products remain illegal under federal law, the TSA now recognizes that some FDA-approved CBD products are capable of providing relief for children who suffer from epilepsy. “To avoid confusion as to whether families can travel with this drug, TSA immediately updated TSA.gov once we became aware of the issue,” said the agency.

The TSA is currently informing their officers on the new policy but this an ongoing process. TSA’s first priority has always been threats to security, with marijuana and illegal substances not being something that’s actively scanned by officials. When encountering a substance that an officer deems as illegal, they will contact a law enforcement officer who will dictate the next steps to follow.

Bulgaria Becomes First EU State To Authorize Sales Of CBD

Bulgaria becomes first EU state to authorize sales of CBD, hopefully starting a trend across the union.

Kannaway, a subsidiary of Medical Marijuana Inc, said it has been issued with a Free Sale Certificate to produce hemp-derived CBD products from Bulgaria’s Ministry of Agriculture, Food and Forestry, and the Bulgarian Food Safety Agency.

The products “comply fully with relevant requirements of the Law on Foodstuffs of Republic of Bulgaria and of Regulation (EC) No 852/2004 of European Parliament and the Council on the hygiene of foodstuffs,” according to documentation, and the compliance also extends to exports.

“We can not find news of any other country in the EU issuing a Free Certificate of Sale for CBD,” a Kannaway spokesperson told media.

The legal status of CBD is still unclear in the EU, as the substance is classed as a “novel food” under regulations, and is being sold without the correct authorisation.

European authorities, having made the announcement, have shown little desire to enforce the law however, and CBD remains on sale in shops across the continent.

Under the regulation, novel foods are defined as not “been consumed to a significant degree by humans in the EU before 15 May 1997, when the first Regulation on novel food came into force.”

According to the paperwork, Bulgaria considers the CBD products it encompasses to be “food product(s) / food supplement(s) [and as such] is/are placed on the market in accordance with relevant legislation of EU and Republic of Bulgaria, and is / are subject to free-sale on Bulgarian market.”

The move means hemp-derived CBD products can be sold in Bulgaria as “traditional foods,” as defined by the The European Industrial Hemp Association.

Kannaway’s vice president for its international segment, Alex Grapov, acknowledged the confusion in the EU, but said the firm is “excited to be on the forefront of companies who are working to dispel the myths and inconsistencies of the industry and expand access with lab-tested hemp-derived CBD products in Bulgaria and soon in other EU nations”.

Representatives from Kannaway lobbied Bulgarian government officials and regulatory bodies earlier this year, pointing to tax revenue and potential health benefits of allowing the products to be sold.

Medicinal cannabis is still on the agenda for the European Parliament, however business has slowed down due to the election period that has now ended. Earlier this year members approved a plan to commit more money and research into medicinal cannabis, and set in motion a plan that would harmonise laws across the bloc related to studying the drug.

Recently, the Parliament also approved another series of proposals to increase the limits for THC in CBD products from 0.2 percent to 0.3 percent, to enter force in 2021.

Halfway through Monday’s episode of The Bachelorette, Hannah casually revealed that Tyler Gwozdz was no longer part of the cast, just one week after he got the season’s first one-on-one date. “Tyler G. had to leave, and that’s upsetting, because I really enjoyed my date with him,” she told the camera — but then quickly moved on.

So, what happened? Tyler was reportedly removed by production amid online comments — and an accusatory Reddit thread –about his behavior. When asked by ET about her connection with Tyler last week, however, Hannah didn’t imply anything was off.

“I think the date went really good,” she shared. “We didn’t see our conversation the first night, but he just has a gentle spirit about him.”

“I feel like he was a really good first date for me, because obviously my first dates don’t always go that well,” she joked, referencing her awkward first date with Colton Underwood on The Bachelor. “[Tyler] was just really calming and nurturing for me to feel confident going into these one-on-ones, because that’s a long time with a person with a very intimate… well… as intimate as you can get.”

The Alabama native had nothing but good things to say about Tyler — but told ET that she would have no problem confronting men if something wasn’t to her liking.

“I handle things directly, in my own beast way,” she said with a laugh, when asked how she’d confront a scandal on her season. “Absolutely [guys felt the wrath of the beast]. They’re probably still in the corner shaking, as they should.”

Hannah also said she has no regrets about everything that went down. “I think about this a lot, because sometimes watching it back or thinking about things, I’m like, ‘Ah, maybe I should have not said that or done this.’ But ultimately, no, because everything that happened, it has got me to where I am now, and I’m stronger,” she said. “I’m more empowered, I’ve grown and I’ve learned so much. So, no.”

“While filming GOT we had a lil break We were to [sic] broke to fly home so we rented a panel van aka UHAUL in Belfast and drove around beautiful amazing IRELAND searching for the greatest pint of Guinness,” he recalled. “Turns out it’s perfect everywhere.”

“So many wonderful people and stories I treasure those times It was the simplest moments I remember the most,” Momoa continued. “I still miss my family. Since his role on “Game of Thrones,” Momoa has gone on to star on the big screen as Aquaman in three feature films. He will reprise his role as the superhero in 2022’s “Aquaman 2.”

Wine Or Weed: Why You Don’t Have To Choose

The “wine vs. weed” debate has been aflame since state cannabis legalization. But Jamie Evans, also known as The Herb Somm, celebrates both wine and cannabis in her work as a San Francisco Bay Area event producer and wine/cannabis sommelier.

“You don’t need to choose. There is room for both at the table,” she says. In her work, Evans encourages a celebration of all things that bloom: wine grapes, cannabis flower, and seasonally fresh food. As a wine industry veteran and a certified specialist of wine, Evans evokes the rich tradition of California viticulture in her exploration of cannabis in her tasting flights and dinners. She applies sensory-based principals of wine tasting to her evaluation and presentation of varietal cannabis, exploring the aroma of each flower’s terpenes, the aromatic and flavorful component of a plant’s essential oils. Evans considers strain, aroma, terpene benefits and effects and pairs them with food and wine.

For example, the aroma found in a flower containing the alpha-pinene terpene (pine needles, wet wood, rosemary, dill) pairs well with a Pinot Gris and Sauvignon Blanc. Evans recommends using a flower vaporizer when pairing cannabis with wine in order to better taste and smell the terpenes unique to the strain. For private dinner events, she sets a classically elegant dinner table and serves each course with a flower pairing and alternates wine, food, flower and vape offerings. “This is what the future might look like. It’s an exciting time for the canna-culinary world.”

Evans acknowledges that while wine has been shown to provide some health benefits, cannabis research continues to be promising as studies show increasingly more medicinal potential on a much broader scale. She also notes that wine and cannabis are both gourmet products, but that there is more application to cannabis than wine including lotions, bath bombs, smoking flower, vaping oil, culinary cannabis applications and more

Like wine country vineyard tourism, cannabis tourism is also beginning its boom, with tourists coming out to California’s cannabis country for farm tours. While most farms are not open to the public due to licensing restrictions, Sonoma County has been most progressive in the development of this new branch of cannabis tourism. “What makes wineries so enchanting is seeing the winemaking process from grape to glass,” says Evans. “With cannabis, it’s from seed to flower consumed. People like to come see the farm and learn about growing practices, building a connection between the farm and the consumer.” Evans says that cannabis farm tours “pull back the curtain to show that it is a plant, a crop, and a natural medicine that has many healing benefits.”  

Concussions And Cannabis

During the 2019 Cultivation Classic, Dr. Ethan Russo of the International Cannabis and Cannabinoids Institute presented his latest research findings on cannabis and traumatic brain injury.

Conventional wisdom is that if the symptoms persist for a year, the symptoms will be present. But as Dr. Russo illustrated during his talk, this conventional wisdom is not always the case and certain things can be done. This includes some promising research into the role cannabis can play in recovery from traumatic brain injuries.

THC and CBD As Recovery Agents

Both THC and CBD are neuroprotective antioxidants, which Dr. Russo observes is a fancy way of saying they help reduce the effects brain damage whether due to trauma or things like strokes or other disease. An antioxidant is something that prevents rust. And according to Dr. Russo, “rust in the brain means deterioration in the brain structures.”

Glutamine is a neurotransmitter that produces an over abundance of glutamine following a head injury that produces glutamate excitotoxicity whereby the cells stimulate themselves to death. This can lead to a neuronal demise after a traumatic brain injury. In Dr. Russo’s research, he’s observed that CBD and THC may help prevent glutamate excitotoxicity. Also, THC and CBD, have been extremely helpful in treatment of chronic traumatic encephalopathy (CTI) symptoms experienced by football players and anyone else engaged in contact sports.

Using Cannabis to Treat Concussions

While one should consult with their personal medical provider before beginning any regime, Dr. Russo offered these overall guidelines for those looking to treat a conclusion with cannabis.

If one choose to inhale cannabis via smoking or vaping, tiny doses should be utilized sequentially with 10 to15 minute pauses. This should help lift “brain fog or allay symptoms such as headache or dizziness. For chronic problems, oral administration of low doses via capsules or tinctures are preferable.

While THC-predominant chemovars can be very effective, in Dr. Russo’s practice, what would be more appropriate would be Type II or III chemovars with low THC and high CBD. In those tepenes that are proven to be particularly effective α-pinene-inene is helpful to reduce SIN- and cognitive impairment; limonene will elevate mood; and carvophyllene will reduce pain and inflammation.

In addition to cannabis, other natural treatments recommended by Dr. Russo include  ginkgo biloba, ginger, and chelated agnesium. Rosé has become crazy popular and spawned a whole line of accessories including “rosé all day” mugs, shirts, towels, etc. The Brad and Angelina fight over their vineyard is STILL in the news and it chills in people’s fridge.  Yes, rosé season is in full swing, and while there’s no absolute need to do anything more than open a cold bottle and pour yourself a glass, why not take your game up a few levels? You need these 4 rosé summer cocktails in your life.

Beginner Level:

Rosé Spritz

Perfect for a long day of drinking, the rosé spritz is little more than four ounces of rosé over ice, topped with soda water. Add an orange twist if you want a little something else.

Rosé Sangria

I’m surprised this version of the summer favorite goes ignored as often as it does, because frankly I think rosé is easily the best sangria. The key is to pick your fruit with some care: I aim more for red berries and stone fruits like peaches and nectarines, while mostly avoiding citrus, as I find that those flavors tend to clash with most of the rosé I like. Watermelon also can make for a nice addition.

While many sangria recipes would call for brandy, I actually like to add a bit of white rum when I make rosé sangria, as again I find it pairs better with the flavors already in there.

Advanced Level:

Rosé Margarita

Getting the balance right on this drink is tricky, especially since I generally prefer a slightly more flavorful rosé, perhaps from Spain or Italy. The choice of tequila is crucial as well; I find younger tequilas tend to work better, or even a joven Mezcal can be delicious. I start with three ounces of rosé, and then add an ounce-and-a-half of tequila, a half-ounce of Cointreau, and an ounce of lime juice, then shake the whole thing and serve over ice; salt is optional, but awesome.

Rosé Negroni

One of the great summer cocktails can take on a whole different dimension when you work rosé in there. Instead of an ounce each of Campari, gin, and sweet vermouth, I use two ounces of rosé, an ounce each of gin and sweet vermouth, and a half-ounce of Campari, then gently stir it and serve it on the rocks. If you use a real dry rosé (like from Provence or somewhere similar), the drink is instantly recognizable as a Negroni, yet somehow different. If you’re feeling especially brave, use a sparkling rosé!
